Please keep following documents with you for OPD registration or admission:
Government-issued photo ID, such as a Driver's license, Voter-ID card, Aadhar Card, passport etc.
Complete list of all prescription and over the-counter medications that patient is currently taking, all the medical reports and discharge card.
Scheme related documents such as Ration Card/B.P.L. list/Caste Certificate/Income Certificate (with photo)/Sandarbh Card/C.G.H.S. Card/MA card/MA Vatsaltya Card/Ayushman Bahrat (PM-JAY) Card etc.
For follow up visit: Show discharge card or old OPD case paper to Registration counter.
In case of emergency, patient and relatives should visit emergency department (E.M.C.S.) Grond Floor, B-Block..
Registration will be done there. If patient will be advised for admission then admission process will be conducted
from E.M.C.S. department by on duty doctor.
Signage board and Path viewer (colour code signage on floor) will guide for the direction of all investigations.

Patient shall fill the OPD registration form prescribed by the Institute and at the designated Case counter to consult the Doctor.
Case Counter staff shall enter the patient data from the OPD registration form in HMIS, which generates patient Identification No. in the system and uploaded Patient Picture in system.
Case paper can be generated on the basis of Original Aadhar card, if available with patient.
Patient takes the OPD case file and goes to consulting Doctor with ECG report.
The doctor inspects the patient and advice for further test required if any
Patient go to cash counter and pay advance through Cash, Credit Card/Debit Card, NEFT/RTGS/UPI or any other option available at Institute and go to lab for investigation report
After review of OPD investigation report Doctor will appraise patient about probable medical for his treatment. Once the patient agrees to admit than he will go to indoor case counter
Case counter staff generate indoor case and patient will go to respective ward for admission.
Ward sister in charge after consult with Doctor advise patient to pay advance for treatment, the patient shall go to Cash counter to pay Medical charges in advance as prescribed by the doctor
If patient is not financially capable to pay medical charges and decides to avail medical assistance under any approved scheme by the Centre, he will be directed to go to Scheme department for authorization of applicability of his relevant scheme in his case paper file.
After your admission process is completed, one of the hospital staff members will escort you to your bed, explain where things are and introduce you to the staff on duty.
You will be given an identity belt with your name and identification details on it. Please wear this all the time whileyou are in the hospital.
After your admission, the on-duty doctor will examine you.
At the time of admission contact number of our control room will be given, you can contact for hospital servicerelated questions.
For any medical queries or concerns, contact on-duty doctor or nursing staff.
Nursing staff and Medical Clinical Coordinator will daily update about the bill to patient /relative (for paid patients only)
Pay the bills at the hospital billing counter only, and do not forget to get the receipt for the same. Billing amount up to 2 lakhs will be accepted in cash, more than 2 lakhs will be accepted through credit card, debit card, RTGS and NEFT.
Please keep patient and close relative's identification proof with you during hospital stay.
Please give your jewellery, wrist watch, mobile, etc. to your relatives after hospital admission, and do not bring other luggage like bags.
Only one companion will be allowed in the room. No relative will be allowed to stay with the patient in ICU. Seating arrangement for such relatives is available outside the ICU.
You may nominate one responsible person to meet the doctor to know about the treatment, during the doctor's round. Many relatives meeting the doctors at different times make it difficult for the doctor to render efficient medical care to all his/her patients.
The nominated person should inform all other family members and other well-wishers regarding your treatment and progress.
The Institute raises Patient Bill for various Medical Services given to patients based on Rate Card approved by the Institute.
Patient’s treatment involves medical operation charges, additional medicines/Investigations/Room Charges over and above package selected by the patient, Implant Cost, ventilator charges and other related charges.
With respect to patient’s medical services are charged to his Patient ID by respective ward staff and settled against the money paid in advance by patient.
On completion of his consultation / medical treatment, doctor of the medical ward decides to discharge the patient from hospital, he shall make necessary remark for discharge and all other medical formalities of discharge shall be completed by medical ward attendant before allowing patient to leave hospital premises.
Ward attendant send discharge patients file to discharge counter. Discharge counter staff checks package mentioned in file will charged in patient ID in HMIS, Ventilator charges, any pending indent request or investigations etc.
Ward attendant will also send the Patient/relative to discharge counter for settlement his patient bill if any. Patient/relative shall go to discharge counter for his bill.
After checking of all required field, discharge counter staff inform in ward for Initiate & confirm discharge date.
In case of Patient treated under scheme then after getting an approval from respective scheme patient will discharged.
After confirmation from ward, discharge counter staff generate final bill of patient and provide one copy of bill to patient if he gets treatment under paid category, LIG & SC-50% scheme otherwise final bill will not be issue to any patients.
For beneficiary of any Government scheme, medicine and travel allowance will be given from the hospital as per rules.
Discharge process may take minimum 2 to 6 hours after intimation of Doctors to nursing staff.
To know and process your refund amount:
Kindly contact billing department at the time of billing process.
You need to fill-up the application form. ID proof and bank passbook will be required to initiate refund.
Amount more than Rs. 2000 (two thousand only) will be refunded in given account number. The process of refund may take 7 to 15 days.
For follow-up: visit your doctor on the day of OPD as mentioned in discharge card.

Provide, to the best of your ability, your medical team with accurate, complete details about past and present illnesses, hospitalizations, surgical procedures, and medications.
Communicate about the need for pain relief with doctors and nurses. Inform your medical team whenever you experience a change in your condition or a problem with your treatment.
Follow the advice and instructions of your doctors, nurses and other hospital personnel concerning your care.
Be courteous to the hospital staff and other patients, minimize noise and keep the number of visitors to reasonable levels.
Do not smoke in the hospital.
Do not damage or remove hospital property or the property of other patients.
Discuss and plan for your post-discharge needs with your medical team early in your hospital stay.
Valuables, such as jewellery, watches, electronic devices, video cameras or other recording devices, Hospital will not be liable for any loss of your valuables.
